YOUTH PROTECTION POLICY COMPLIANCE CHECKLIST

Complete a separate form for EACH youth program/camp occurrence and submit at least three (3) months prior to the event/program start date.

Youth Protection Training – Assigned by the Office of Institutional Compliance to University staff and completed online

Use the template provided via the Youth Program/Camp Notification Form.
Include program name, Notification Form number and event date(s) on this list.
☐ Youth Protection Training for all designated individuals must be completed five (5) days before event start date
For University staff/student workers: Online course will be assigned by the Office of Institutional Compliance to complete online.
For volunteers/students/third-party agents: Training and exam will be facilitated by the identified University event coordinator/point of contact using state-approved training materials supplied by the Office of Institutional Compliance.
All programs that last four (4) or more days and have twenty (20) or more Youth participants. file the
Department of State Health Services Campus Program for Minors Form
within five days of event/program start.

Background Check and Information Form
for all designated individuals
Submit the form with the link above for each designated individual using the applicable cost center information for the program/event.

Track the status of the Criminal Background Check (CBC) and verify the CBC cleared five (5) days prior to the start of the program. Contact

☐ Liability Insurance
All Youth programs must carry accident and liability insurance with coverage at least equivalent to the Camp Insurance Program offered by the U.T. System Office of Risk Management. Any purchase of insurance must also comply with the requirements of
Regents' Rule 80601
.
If an accident and liability insurance, other than the UT Systemwide Camp Insurance Program, is utilized, proof of the alternate coverage must be provided seven (7) days prior to the event occurrence.
