GRADUATE SCHOOL EXCEPTION REQUEST FORM

Questions marked with a * are required
Exceptions to our rules are sometimes necessary to help students earn their degree and achieve their goals and aspirations. Graduate Program Directors or designated program staff members can use this form to request an exception – please find a full list of available options in the multiple-choice question below. Students who submit this form may be subject to disciplinary actions.

Most rules are set by faculty; for the Graduate Dean to provide an exception there must be appropriate justification given.

Because Individualized Development Plans (IDPs) help students make progress toward overarching milestones like theses and dissertations, all students who are pursuing a thesis or dissertation must complete an Individualized Development Plan (IDP) at least once a year. Most exceptions for students in thesis and dissertation programs require at least one IDP.

Please do not upload FERPA-protected material via this form. Specifically, per the UTEP Center for Accommodations and Support Services (CASS) policy handbook: “Treat information about a student’s disability and accommodation confidential as per FERPA requirements” If such an accommodation is relevant to the waiver/exception request, it should be mentioned in the program director or Graduate Program Committee letter uploaded with this form, so that the Graduate Dean may take appropriate follow-up steps.
This form must be submitted by a faculty program director or program staff member. This form may NOT be submitted by students. Please confirm below.
