This survey will collect information about your concern.

It is assumed that your concern is NOT related to a grade grievance or a matter of discrimination, UTEP has a separate procedure for voicing those concerns.

Further it is assumed you have spoken to the parties involved and their supervisor. Often that is the Department chair of the party involved which may differ from your major. 

This form captures some basic facts about your concern and will be presented to a responsible person in the Dean's office. You will be asked for contact information so you can be contacted for further investigation.
